First Impressions

This week's competition, in which you have to identify a book from its opening sentence, is from a work by a doctor:

"Dearest Cousin Mikhail Mikhailovich, A few days ago I had the honour and pleasure of receiving your letter."

The winner receives a half-case of wine. (Please include a telephone number to arrange delivery.) The closing date is June 15.
