Colin Norris, who trained at the University of Dundee, killed four patients while working at a Leeds hospital by administering overdoses of insulin. He is now serving four life sentences. An inquiry commissioned by NHS Yorkshire and Humberside found that during his training he was frequently absent from clinical placements and "had difficulties dealing with authority". The inquiry said it was concerned that the reference supplied by the university indicated that he was "an average student" who was polite and capable. Dundee said that its reference "was consistent with normal practice across the sector", adding that at the time "no significant concerns had been raised" about the nurse.
