Position Focus:

The Exercise Therapist will be responsible for implementing a mobile-based cardiac rehab intervention at the Yale site after participants are identified and enrolled at either of the 2 Yale-affiliated hospitals.

Essential Duties

  1. Perform baseline (in-hospital) exercise therapy counseling visits with study participants. 
  2. Prescribe personalized home exercise programs, including setting of activity targets based on participant’s needs and reviewing home exercise data of all research participants.
  3. Make periodic (weekly) phone calls with participants to review exercise data and to provide counseling on promoting physical activity, for the duration of the study intervention.
  4. Be available for periodic (as-needed) electronic communications with participants through the Moving Analytics mobile health (mHealth) software application. 
  5. Responsible for monitoring all participants through the mHealth dashboard for 3 months and report any identified adverse events or symptoms to the PI and research team.

Required Education and Experience

Bachelor’s Degree in Physical Therapy or similar (e.g., Exercise Kinesiology) from an accredited program of study.

Required Skill/Ability 1:

Ability to prescribe a personalized home exercise program, including setting of activity targets based on participant’s needs and reviewing home exercise data of all research participants.

Required Skill/Ability 2:

Ability to learn and administer mobile health software to research participants. Working knowledge of wearable activity monitoring devices (e.g. FitBit), portable electronic tablets, and mobile health technology features, including chat messaging and data management through the web-based dashboard.

Required Skill/Ability 3:

Ability to work independently and with confidential matters while interacting with patients and health care providers.

Required Skill/Ability 4:

Strong interpersonal skills and the ability to interact comfortably with research and clinical staff.

Preferred Education, Experience and Skills:

Master’s degree in Physical Therapy or similar field, ≥1 year experience in practicing physical therapy or rehabilitation.
