Position Focus:

In 1971, Yale University began a unique and historic experiment in providing health services to its faculty, staff and students through a multidisciplinary health maintenance organization located on campus. Our state-of-the-art facility at 55 Lock Street, built in 2010 is where our members receive most of their care. It is a 144,000 square foot medical facility with over 90 exam rooms, a 24/7 Acute Care Department, a 17-bed inpatient facility, a diagnostic imaging suite and a full-service retail pharmacy. It houses our 150+ providers, including 111 primary and consulting physicians.

We are currently seeking an RN to join the Student Health Team. The Primary Care Team Nurse assumes responsibility for triage, assessment, care-coordination, follow up and transitional care, clinical evaluation and clinical decision making for the assigned population and unit. Assumes responsibility for prioritization of care and disposition of patients from both telephonic encounters as well as scheduled and unscheduled encounters. Compliance with nursing principles, medication safety and adherence to scope of practice is a fundamental expectation. The Clinical Nurse utilizes the nursing process of assessment, planning, intervention and evaluation while caring for our patients maintaining confidentiality and dignity. The Clinical Nurse provides safe, effective evidenced based care tailored to the individual needs of the patient and family. As a core member of the care delivery team, the Clinical Nurse partners with the team to facilitate the coordination of patient care at the unit level utilizing the principles of patient/family centered care. Ensures clinical policies and procedures are consistently followed, initiates new policy when appropriate. Role includes initiatives and services that support organizational and strategic goals. RN role serves to support clinicians and all members of the care team to prioritize and anticipate patient care needs. Fosters ongoing improvements of service and a culture that prioritizes safety. Role requires the ability to interface with key clinical partners, ability to make difficult decisions and facilitate resolution.

Essential Duties

1. Performs physical assessments of patients, including triage duties and advises on treatment or course of action by having knowledge of nursing care standards and principles. 2. Develops, implements, and manages nursing care plans in collaboration with physicians. 3. Interviews patients and records medical history to establish data for treatment plan. 4. Plans and coordinates the admission and discharge of all patients. 5. Instructs patients and families in proper treatment and advises patients on health maintenance and disease prevention. 6. Performs patient care procedures by following appropriate policy and procedure. 7. Records patients’ medical histories and symptoms, operates medical machinery, administers treatment and medications, and coordinates patient follow-up and rehabilitation; assures all procedures for maintaining patient confidentiality are maintained per HIPAA guidelines. 8. Identifies education needed to assure competent delivery of patient care for oneself and others in the department. 9. May perform other duties as assigned.

Required Education and Experience

Graduation from accredited school of nursing.

Required Skill/Ability 1:

Previous nursing expertise with demonstrated ability to multi task and prioritize using the nursing process in a high volume out patient environment. Competent clinical triage skills and critical thinking skills.

Required Skill/Ability 2:

Demonstrated evidence of maintenance of professional skill and competency, attendance at educational programs such as seminars and workshops that are appropriate to the field. Demonstrated active membership in professional organizations.

Required Skill/Ability 3:

Outstanding interpersonal skills, ability to communicate and excellent written skills. Computer proficiency as it relates to the use of an EMR,

Required Skill/Ability 4:

Excellent attendance, punctuality and reliability.

Preferred Education, Experience and Skills:

BSN strongly preferred. Demonstrated Nursing experience with at least one year in an In-patient hospital unit in the specialty that is appropriate to the position. Experience in an ambulatory care, critical care, SICU, PICU, primary care (sub-acute facility) or a college health center. EKG interpretation skills.  Proficiency in Epic or Clear Triage.

Required Licenses or Certifications

Licensure as Registered Nurse in the State of Connecticut or Connecticut license eligible. Current CPR certification.
