As Advanced Practice Registered Nurse, assist in the evaluation and management of new and existing patients with diabetes and related metabolic conditions at the Yale Diabetes Center. Patients will span the entire spectrum from diet controlled patients with Type 2 diabetes to Type 1 diabetic patients on intensive insulin regimens, including insulin pumps. In this capacity, the Diabetes APRN is an integral part of a collaborating team composed of the APRNs/PAs, diabetes attendings, and endocrine fellows. This position is part-time negotiable and there is some flexibility to work hours, including possible 4-day work weeks. There is also the possibility of some inpatient work on the diabetes consult service at Yale-New Haven Hospital.
- Examines patients to collect information about their physical condition and establishes a clinical diagnosis.
- Conducts physicals, provides treatment, formulates a health care plan and counsels patients on various health practices.
- Encourages compliance with treatments plan through education, assessment of patient understanding and regular patient monitoring.
- Provides reasonable and prudent medical care, practices appropriate management, and uses sound clinical judgment based on best practice standards and evidenced based medicine.
- Obtains, compiles, and records patient medical data, including health history, progress notes and results of physical examination.
- Provides preventive health practices to patients to maintain and improve health and prevent illness.
- Coordinates patient care including ordering of laboratory tests and radiographic studies.
- Interprets diagnostic test results for deviations from normal.
- Prescribes therapy or medication as necessary.
- May perform other duties as assigned.
Required Education and Experience
Master’s Degree from an accredited Advanced Nurse Practitioner program or graduation from a Physician Assistant (PA) Training Program accredited by the American Medical Association Council on Medical Education.
Required Skill/Ability 1:
Demonstrated knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in the areas of hypertension, hyperlipidemia, and obesity.
Required Skill/Ability 2:
Passion for delivering outstanding, state of the art, patient centered care in an academic environment. Knowledge of health education theory and practice. Knowledge of general clinical operations and procedures.
Required Skill/Ability 3:
Highly efficient and organized work habits. History of reliability and impeccable attendance. Demonstrated ability to make appropriate independent decisions and to seek physician involvement when necessary.
Required Skill/Ability 4:
Ability to maintain quality, safety and/or infection control standards. Knowledge of related accreditation and certification requirements.
Preferred Education, Experience and Skills:
Current licensure and prescriptive privileges in the State of Connecticut. Experience in diabetes management. At least 1 years of experience managing in an outpatient setting. Knowledge of insulin pumps and continuous glucose monitoring.
While performing the duties of this job, the work involves sitting, standing, walking, bending, and occasional slight lifting. The employee occasionally lifts and/or moves up to 50 pounds when transferring a patient from chair or wheelchair to exam table and turning patients for examination. Proper body mechanics must be used when maneuvering patients.
