General Summary:

Reporting to the Brancati Center Program Manager, the DPP Program Director and the Director of Community Partnership, this position will serve as the lead Lifestyle Coach in delivering the National Diabetes Prevention Program in community-based settings.  Under the direction of the DPP Program Director, the Lifestyle Coach will participate in all phases of program implementation, operations oversight, and data monitoring and reporting. This position will require administrative supervision of coach assistants and community health workers.  The Lifestyle Coach will work with Brancati Center staff to ensure the DPP achieves program outcomes. This position will directly interact with overweight, pre-diabetic participants (community members) of various age and race, church and community leaders and members, physicians, and other payer communities and referral networks related to the DPP.

Essential Functions:

Under the direction of the DPP Program Director, the Lifestyle Coach will lead and oversee all aspects of the DPP groups, as well as special intervention projects and initiatives relating to the DPP. The Lifestyle Coach will assist the Program Director with DPP curriculum reviews, revisions and the development of supplemental group materials when needed. The Lifestyle Coach may assist with conducting quality assurance measures for evaluating lifestyle coaches. The position may also supervise and provide support and guidance to lifestyle coaches and coach assistants as hired. The Lifestyle Coach will also work in partnership with the DPP Coordinator to carry out all administrative and programmatic tasks related to DPP implementation.

DPP Program Development

Prior to DPP group implementation, the Lifestyle Coach will (in collaboration with the Brancati Center team):

  • Work with the Directors and staff to assist in the program development phase of the DPP in-person groups and potentially the virtual DPP.
  • Attend meetings with community partners to establish new DPP groups.
  • Take part in community events to enhance awareness of the Brancati Center’s DPPs.
  • Assist with conducting behavioral interviews during participant recruitment for the DPP program.
  • Attend DPP Open House events to screen and enroll participants into DPP programs.
  • Become familiar with the National Diabetes Prevention Program curriculums and the National Diabetes Prevention Recognition Program Standards.
  • Become trained as a certified CDC Lifestyle Coach.
  • Organize program materials and delivery of the DPP program with adherence to a CDC-approved curriculum.
  • Help establish the program structure and timeline for various stages of DPP implementation: group delivery, coach oversight, data monitoring and reporting.
  • Help to create internal tracking systems for evaluating and reporting program outcomes.
  • Become familiar with REDCap database.

Program Implementation

During program implementation, the Lifestyle Coach will (in collaboration with the Brancati Center team):

  • Serve as lead coach to deliver the lifestyle change program during program implementation.
  • Facilitate lifestyle groups as related to weight loss, physical activity, and behavior change.
  • Help participants develop and maintain skills needed to adopt healthy lifestyle habits.
  • Register, schedule and document in EPIC for relevant participants (Medicare & Medicaid).
  • Utilize teaching tools specific to participant populations served.
  • Support participants by providing information, encouraging progress, and addressing challenges.
  • Collaborate with the Community Health Worker to identify and resolve participant barriers.
  • Demonstrate problem-solving skills as they relate to participant care activities, and acts as a role model for other lifestyle coaches.
  • Assist with development of supplemental class materials for special group topics. 
  • Deliver and follow the NDPP curriculum and adjust to meet the needs of the participants. 
  • Participate in behavioral case management meetings to evaluate participant behavior, identify problems, and implement corrective actions.
  • Supervise and assist the lifestyle coaches with launching each group.
  • Work with the DPP Coordinator in planning coach collaboratives
  • Provide support and guidance to the coaches and participate in routine coach collaboratives to monitor the lifestyle program participants and program outcomes. Ensure adequate class coverage with staffing/group schedules.
  • Monitor retention and commitment of program participants.
  • Review program data to ensure it meets quality performance standards.
  • Assist the Program Director with creating and implementing quality assurance measures to ensure coach compliance with CDC’s Diabetes Prevention Recognition standards.  
  • Consult with Directors to review program goals, timelines and procedures for accomplishing the project.
  • Participate in weekly DPP team meetings with Brancati Center staff and with the DPP Director.
  • Ensure compliance with JHU operational policies, applicable laws and regulations and those governing privacy and data security (HIPPA regulations).

QUALIFICATIONS

Minimum Education & Experience:

  • Bachelor’s Degree in Nutrition, Health Science, Public Health, Behavioral Psychology, Kinesiology, or related degree.
  • At least one year of experience in conducting health education classes and/or behaviorally oriented groups ,clinical experience and/or knowledge of prediabetes, diabetes, hypertension, and/or obesity; familiarity with motivational interviewing and knowledge of the principles of behavior change, and cultural competency regarding the needs of minority populations.

Special Skills and knowledge:  

  • Extensive knowledge of Diabetes Prevention Program lifestyle intervention
  • Able to deliver behavioral intervention specific to the National Diabetes Prevention Program
  • Excellent computer skills:  Microsoft Office, EPIC 
  • Excellent organizational skills with specific attention to detail and data collection. 
  • Able to work independently as well as collaboratively as a team member.
  • Outstanding verbal communication and interpersonal skills with participants and other team members.
  • A friendly demeanor with strong customer service skills.
  • Flexibility to work with people from all walks of life.
  • Familiarity with the public health community.
  • Has access to reliable transportation.
  • Able to work some evenings and Saturdays with variable work conditions.

Physical Requirements:

Physical Requirements: Sitting in a normal seated position for extended periods of time. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity to manipulate objects with fingers rather than with whole hand or arm, for example, using a keyboard. Communication skills using the spoken word. Ability to see and hear within normal parameters. Ability to move about workspace and to lead group classes.  Ability to drive to multiple sites in a timely fashion.
