Occupational Summary

The Patient Navigator assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes.

Work Performed

The Patient Navigator assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes.The Navigator coordinates research data, community physicians, and Duke Health System resources into a seamless model of access and care benefiting patients, participating physicians, and family members. Perform other related duties incidental to the work described herein.

Education

Bachelors degree in a health-related field required.Masters degree a health-related field preferred.

Three years of experience in patient education, nursing, planning, case management, or similar experience.
