Job Summary

  • Provides direct patient care, by supporting a General Medicine practice in an academic setting, through care coordination for adult patients with complex illness, in the primary care setting under minimal supervision. 
  • Independently manages a schedule of patients including health maintenance exams, return visits, same day urgent care and discharge visits
  • Collaborates with members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services. 
  • Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patients’ health status. 
  • Integrates evidence-based clinical guidelines, preventive guidelines, and protocols, in the development of individualized care plans that are patient-centric, promoting quality and efficiency in the delivery of health care.   
  • Provides targeted interventions to avoid hospitalization and emergency room visits.
  • Coordinates care across settings and helps patient/families understand health care options.

Responsibilities*

MAJOR DUTIES AND RESPONSIBILITIES:

  • Provide direct patient care as an independent part of a team by supporting a General Medicine practice 
  • Perform initial and ongoing assessments, diagnostic evaluation, consultation and treatment of adult patients as delegated by an attending physician.  Assesses over time the health care, educational, and psychosocial needs of the patient/family.   Uses standardized assessment tools such as depression screening, functionality, and health risk assessment.
  • Collaborates with PCP, patient, and members of the health care team, including continuum of care settings and community.  Responsible for watching the comprehensive individualized plan of care and targeted interventions.  Continually observes patient/family response to plan of care, and revises the care plan as indicated.
  • Supports patient self-management with a focus on empowering the patient/family to build capacity for self- care through clinic visits and other complex care needs. 
  • Implements systems of care that facilitate close monitoring of high-risk patients to prevent and/or intervene early during acute exacerbations.
  • Implements clinical interventions and protocols based on risk stratification and evidence-based clinical guidelines. 
  • Coordinates patient care through ongoing collaboration with PCP, patient/family, and other members of the health care team. 
  • Demonstrates excellent written, verbal, and listening communication skills, positive relationship building skills, and critical analysis skills. Identifies the targeted high risk population within practice site(s) per PCP referral, risk stratification, and patient lists.
  •  Maintains required documentation.
  • Reviews the current literature regarding effective team engagement and communication strategies, clinical behavior change strategies and incorporates into clinical practice.

SKILLS AND ABILITIES:

  • Demonstrated ability to perform health assessments including patient history, physical examinations, diagnostic tests and appropriate interventions, prescriptions and/or referrals.
  • Demonstrates customer focused interpersonal skills to interact in an effective manner with practitioners, the interdisciplinary health care team, patients, and families with diverse opinions, values, and religious and cultural ideals.
  • Demonstrates ability to work autonomously, with minimal supervision and be directly accountable for practice.
  • Demonstrates ability to influence and negotiate individual and group decision-making.
  • Demonstrates ability to function effectively in a fluid, dynamic, and rapidly changing team environment.
  • Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, critical thinking, analysis skills and decision-making, priority setting, work delegation, and work organization.
  • Demonstrates ability to develop positive, longitudinal relationships and set appropriate boundaries with patients/families.

Required Qualifications*

NP

  • Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
  • Current specialty certification to practice as an Adult Gerontology Primary Care Nurse Practitioner or Family Nurse Practitioner.
  • Graduate of an accredited Nurse Practitioner program.
  • Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).   Possession of a current licensure to practice as a RN and as a Nurse Practitioner in the State of Michigan.
  • Three (3) years of experience with adult patients in primary care or ambulatory care within the past five years

PA

  • Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
  • Certification by the National Commission on Certification for Physician Assistants (NCCPA).
  • Current license to practice as a Physician Assistant in the State of Michigan.
  • Three (3) years of experience with adult patients in primary care or ambulatory care within the past five years

Work Schedule

Hours/Week:  40Shift/Hours/Days: Schedule is Monday through Friday, some Saturday hours, with variable clinic schedule including evening hours.  Needs to be available for patient care Monday through Friday (all 5 days) and Saturday with variable shift lengthsLocation: Brighton Health Center Primary Care NP MEDICAL SUBSPECIALTY TIER 2
