Adult Medicine APP
The Adult Medicine APP provides patient care for adults and students admitted to the Inpatient Care Facility (ICF) under the supervision of the Medical Director of the ICF at Yale Health (approximately 50% time, 1/2 days, Monday - Friday). Position also provides care for adults in the department of Internal Medicine at Yale Health, as part of a multidisciplinary care team (approximately 50% time).
Essential duties for the Inpatient Care Facility (ICF) include: completion of admission assessments and EMR notes for each new Internal Medicine admission within 24 hours of patient arrival; daily rounds on all inpatients; discuss patient progress daily with the Medical Director or Attending Physician as necessary; Charts according to ICF policies and procedures; initiates diagnostic and laboratory testing as needed, reviews and interprets results, enters necessary orders in the ICF medical record; works collaboratively with the Care Management Department to ensure seamless transition to home; assesses acutely ill patients and transfers patients to YNHH as needed, in consultation with the Medical Director or Attending Physician; attends weekly Discharge Rounds with ICF staff and actively participates in other ICF meetings as necessary; interacts with nursing and other staff in a professional, cooperative and supportive manner and is available for consultation; participates in the development policies, procedures and clinical protocols that improve the efficiency and quality of care provided in the ICF.
Essential duties for Internal Medicine include: comprehensive whole-person primary medical care to an assigned panel of patients including preventive care and physical examinations, assessment and management of acute issues, appropriate monitoring of chronic illness, and assessment of emotional health and wellbeing; recommend referrals to specialists as needed, consulting as needed with physician team members; provide rapid access care, preoperative assessments and supportive care as needed for non-panel patients on the primary care team; participate in cross-coverage for absent primary care team members; commit to medically appropriate care that is patient centered, evidence-based, compassionate and cost-effective; commit to thorough patient education about treatment options, pros and cons of various choices and shared decision-making with patients and family; work collaboratively with team members to develop and maintain team based care by participating in team meetings, rotating leadership responsibility and contributing to performance improvement projects.
The Adult Medicine APP adheres to departmental and organizational standards of patient service and responsiveness, including but not limited to timely and appropriate communication of test results and timely response to patient messages; adheres to organizational standards of medical documentation within the Yale Health electronic health record (Epic) including but not limited to, timely completion of notes, appropriate documentation of non-visit-related transactions, appropriate and timely management of Inbasket; participates in rotation for weekend and holiday coverage in Acute Care (our 24/7 urgent care setting); participates in organization-wide committees and project work and abides by Yale Health Code of Conduct as well as all relevant policies and procedures.
- Examines patients to collect information about their physical condition.
- Obtains, compiles, and records patient medical data, including health history, progress notes and results of physical examination.
- Conducts complete physical examinations; provides medical evaluation and treatment to patients; develops treatments plans and monitors patient’s progress.
- Advises patients on results of laboratory tests and recommends referrals to specialists or other medical facilities.
- Collaborates with physicians, nurses, counselors and other medical staff to provide the best medical care.
- Provides preventive health practices to patients to maintain and improve health and prevent illness.
- Coordinates patient care including ordering of laboratory tests and radiographic studies.
- Prescribes therapy or medication with physician approval.
- May perform other duties as assigned.
Required Education and Experience
Graduation from a Physician Assistant (PA) Training Program accredited by the American Medical Association Council on Medical Education or Master’s Degree from an accredited Advanced Nurse Practitioner program.
Required Skill/Ability 1:
Expert clinical assessment, interviewing, counseling, problem-solving and crisis intervention skills. Well-developed oral and written communication, data analysis, organizational and training skills. Excellent chart and problem oriented medical recordkeeping.
Required Skill/Ability 2:
Demonstrated excellent attendance, punctuality and reliability.
Required Skill/Ability 3:
Demonstrated ability as a Hospitalist and experience in Primary Care.
Required Skill/Ability 4:
Demonstrated teamwork, flexibility and collaborative skills.
Required Skill/Ability 5:
Prescriptive privileges or eligibility in the State of Connecticut, federal DEA license, and CT DCP license for prescription of controlled substances. BLS and ACLS certification.
Required Licenses or Certifications
CT APRN license (or eligible) with ANA certification OR CT PA license (or eligible).
While performing the duties of this job, the work involves sitting, standing, walking, bending, and occasional slight lifting. The employee occasionally lifts and/or moves up to 50 pounds when transferring a patient from chair or wheelchair to exam table and turning patients for examination. Proper body mechanics must be used when maneuvering patients.
